Who is responsible

The controller is 3D Body Contouring Ltd, trading as The Fat Freezing Specialist. I am Celeste Savage. The clinic is at Sandown Park, Tunbridge Wells, Kent TN2 4RP.

What I collect, and why

The free assessment form asks for your name, email, phone number, the area you want to treat, a few questions about your situation, and any health information that might affect suitability. That health answer is special category data under Article 9 of the UK GDPR.

I use it only to assess whether treatment is appropriate and to reply to you. I do not use it for marketing lists, and I do not sell it.

If you send a photograph on WhatsApp after the form, that photo is used for the same assessment. WhatsApp is a Meta service; their own terms apply to that message once it leaves this site.

The lawful basis

Contact details are processed on your consent, which you give with the privacy tick on the form (UK GDPR Article 6(1)(a)).

Health information is processed on your explicit consent (Article 9(2)(a)). You can refuse that tick and still telephone the clinic if you would rather not put health details on a form.

If you later become a client, I keep the records needed to treat you safely. That is a separate clinical record, not this website form.

Who else sees it

The form is hosted on Vercel. The answers are sent to GoHighLevel, the clinic CRM, so I can see the enquiry and reply. The health answer is stored in a note on your contact record, not in a public field.

I do not create extra custom fields in the CRM for medical answers. I do not put those answers in platform logs.

Online bookings are managed through GoHighLevel and card payments are processed by Stripe. Where you authorise a card to be kept on file, Stripe retains the payment details securely and the clinic can use the saved payment method for authorised no-show or late-cancellation charges. I do not store full card numbers on this website.

Photographs on this website are served by ImageKit. They are existing client results, published with permission, not your assessment photo.

The map on the contact page loads from Google when you reach that embed. Google Analytics is a separate service and only loads if you accept analytics cookies.

How long I keep an enquiry

Assessment enquiries are kept while I am dealing with them, and for up to 24 months if we do not go on to treatment, so I can see a returning enquiry in context. You can ask me to delete an enquiry sooner.

If you become a client, treatment records are kept for as long as a clinic reasonably needs them, which is longer than a website enquiry.

Cookies And Analytics

Google Analytics 4 helps me understand visits, popular pages and enquiry actions. It only loads after you choose Accept Analytics. Rejecting analytics does not affect the assessment form or booking. You can change or withdraw your choice at any time using Cookie Settings in the footer.

Analytics receives limited page information and generic enquiry events. It does not receive your name, email, telephone number, assessment answers, health information or photographs. URL query strings and fragments are excluded, and automatic form tracking and advertising personalisation are disabled.

If accepted, Analytics uses first-party _ga cookies with a maximum lifetime of 60 days. The property retains user-level event data for two months. Google processes Analytics data and may process it outside the UK. Google's privacy policy explains its handling of that data: policies.google.com/privacy.

Your cookie choice is stored on your device for 180 days. Withdrawing consent stops Analytics and removes its cookies from this site. The hosting platform may use strictly necessary cookies, and the WhatsApp prompt remembers a dismissal in local storage. Booking, payment and map providers operate their own services as described above.

Your rights

You can ask for a copy of what I hold, ask me to correct it, ask me to delete it, or withdraw consent for an enquiry. Write to the email above. You can also complain to the Information Commissioner's Office at ico.org.uk.
